Arterial CABG in Triple Vessel Coronary Artery Disease.

Mr. S. D. 64yrs male was diagnosed Coronary Artery Disease 1 month back by Symptoms of Angina & positive exercise test. His Coronary Angiography showed that he had five vessel blocks in his Coronary arteries. He has borderline diabetic & hypertension, non- smoker with BMI at 20 / mild obesity. His Coronary Artery blocks occurred due to his lifestyle & risk factors & strong family history. He underwent total arterial revascularisation ( TAR )
( Arterial CABG ). Utilising
1) Left Internal Mammary Artery ( LIMA) to Bypass LAD & D1 Coronary Arteries .
2) Radial Artery conduit used to Bypass 2 obtuse Marginal Coronary Arteries branch of Circumflex Artery on back of heart.
3) Right Internal Mammary Artery ( RIMA) used to Bypass Right Coronary Artery block. This ( Arterial CABG) was done by Senior Heart Surgeon Dr. Pavan Kumar at Lilavati Hospital some time back. Patient had smooth recovery & discharged on 7th post operation day.Discussion :-Arterial type CABG(ACABG) is advanced technique of Coronary Artery Bypass Surgery. In this technique no Leg veins are used as graft / conduits to Bypass Coronary Blocks. Only Arteries like LIMA, RIMA & Radial Artery are used as they have least tendency to develop graft atherosclerosis & graft failure in Long term beyond 10 years. Many International studies have shown excellent patency rates of 90% & above in arterial grafts than venous graft. Now a days more & more surgeons use arterial grafts for their Coronary Disease patients for better long term outcome.
