Complex Heart Surgery / Coronary Artery Bypass Surgery After Brain Stroke
Mr. V. P. S. 60-Year-old gentleman suffered a brain stroke in 2019 due to Intracerebral bleeding he underwent brain surgery to drain out blood/water from the brain by V. P. Shunt. He recovered completely in 3 months time. Early this year(2020) in January he suffered major heart attack where his heart pumping function came down to 30%. Coronary Angiography showed Severe Triple vessel disease with left main occlusion of 90%. He also had a 5-year-old pacemaker implanted for sick sinus syndrome in his heart.
Due to left main artery occlusion he has advised Coronary Artery Bypass Surgery, which was carried out successfully at Lilavati Hospital by Consultant Cardiac Surgeon Dr. Pavan Kumar in February 2020. The patient recovered well & discharged on 7th post-op-day.
Discussion – This case comes in a very high-risk category for coronary artery bypass surgery. Previous Brain Stroke, paralysis, brain infarction, V.P.Shunt can result in permanent brain damage during heart surgery. In such cases brain protection during heart surgery is of vital importance. Special protocols to keep blood pressure stable, Brain function monitoring & anesthesia level monitoring during surgery are important aspects for successful outcome. I have now large series of such complex comorbidities patients to undergo Coronary Bypass Surgery safely with over 25 years of my experience. It can be done successfully with a good outcome.
