Patient A.S. 49 yrs is a female small built lady . She had Rheumatic Aortic stenosis & Rheumatic Mitral stenosis since last 15 years . Which gradually progressed over the years & since last 1 year she became more & more symptomatic like breathlessness ( NYHA Class IV ) increased developed swelling in feet ( edema) , water in abdomen ( Ascites) & water in lungs. She was hospitalised In condition of heart failure. She was put on aggressive anti-failure treatment .
Investigation like Echo-Cardiography suggested, critical narrowing of Aortic valve
( < 0.9cm2) Aortic stenosis ( AS ) Severe Mitral stenosis ( MS) with fixed orifice leakage. Severe enlargement of heart chambers & severe pulmonary hypertension , poor heart function. She was taken up for heart surgery by Senior Heart Surgeon Dr. Pavan Kumar at Lilavati Hospital in early July 2019. She underwent Double Heart Valve Replacement , Open Heart Surgery ( DVR ) (OHS) . Involving replacement of Aortic Valve Replacement (AVR) & Mitral Valve Replacement ( MVR ). Aortic Valve & Mitral Valve were replaced by Mechanical Artificial Valves as patient was young. The Open Heart Valve surgery went off vey well with Successful recovery.
Discussion – Double Heart Valve Replacement ( DVR ) Heart operation is complex heart operation, requires technically advanced heart surgery skills to get desired success. Specially these hearts are sick with poor heart pumping capacity. Long cardiopulmonary Bypass time compromised Liver function, Lungs status. Operative & post operative recovery can be stormy & complicated & death rates are as high as 10-15% in international risk charts.
