L. V. Aneurysm repair with coronary artery bypass surgery ( CABG )

M. J. 56 yr old male suffered from coronary artery disease since many years & had suffered
Heart attack 3 years back. He recovered & was medical management. He also had diabetes & Smoker since last many years. Recently he again suffered heart attack one month back. Coronary angiography showed 100% occlusion of LAD & RCA with 90% block in circumflex artery. Echo Showed heart function 30 % with left ventricular apical aneurysm. He was advised coronary artery bypass surgery ( CABG) & L. V. Aneurysm surgery.
He underwent CABG x 2 with Left ventricular aneurysm repair recently by Cardiac Surgeon Dr. Pavan Kumar at Lilavati Hospital successfully. His operation and post operative period was smooth & he recovered completely with improved heart function of 45%.
Discussion – Left ventricular aneurysm is serious complication following heart attacks. It happens in 2 – 5 % cases following major heart attacks. This causes heart muscle damage permanently and heart contractility is severely impaired and compromised. These patients suffered from heart failure ,Heart arrythmias or angina. 10 – 20 % Patients die of these complications if not operated upon in time. Surgery for Left ventricular aneurysm is complex procedure.
1)It involves excision / resection of aneurysm dead left ventricular muscle area & remodeling Left ventricle if aneurysm is large & contains clots in it.
2)Secondly aneurysm repair procedure is approximation & exclusion of aneurysm sac without Excision when there is no clot inside and aneurysm sac is well defined borders.
In this patient left ventricular aneurysm repair was carried out along with coronary bypass surgery. Left ventricular aneurysm surgery requires experience & skill for good outcome as it has high chance of complications and risk during operation and after operation.
