ASD heart surgery in Adult female<\/p>\n
Mrs. T. Q. 50 yrs old female was diagnosed \u201c Congestive heart failure with fast & irregular heart beats\u201d. Her complaints of breathlessness were off since last one month. She was admitted in ICU of BSES hospital for anti-failure treatment. On investigations ECG showed fast atrial heart fibrillation, X\u2019Ray chest showed gross heart enlargement & congested lungs. Cardiac Echo showed very large \u201cAtrial Septal Defect of secundum \u201d verity size 3.5cm2 with bi-directional shunt & severe pulmonary hypertension. After adequate anti-failure treatment repeat Echo-cardiography continued to show severe pulmonary hypertension & bi-directional shunt across ASD. To confirm inoperability Cardiac catherization was performed, which showed oxygen step up in right atrium & fall in pulmonary hypertension on oxygenation. These are signs of operability.<\/p>\n
She was taken up for \u201cOpen Heart Surgery ASD repair\u201d by Dr. Pavan Kumar. Intraoperative pulmonary artery pressure was 75mm \/ 30 mmHg which came down 55\/20 mm Hg after surgery. Patient had very large atrial secundum septal defect of 4cm2 size ,which was repaired using \u201c Dacron synthetic patch\u201d. Irregular heart rhythm of atrial fibrillation was also converted to normal heart rhythm by cardioversion. Operation was successful & patient had smooth post operative recovery.<\/p>\n
Discussion \u2013
\nSuch late presentation of Atrial Septal defect ( Hole in heart ) In fifties is rare & gives classic heart failure presentation. Many times these cases are un-operable due to super high pulmonary artery pressure. Echo cardiography may not give proper operability status. It is very important to perform Cardiac catheterisation to get proper diagnosis of operability as was done in this case. This operation of hole in the heart of adult female was successfully done & will give new lease of life to patient.<\/p>\n","protected":false},"excerpt":{"rendered":"
